Employment Forms

Welcome to your new position with Belmont Public Schools! We hope your career with us will be a long and happy one. Below you will find all of the required forms and information you will need to begin your employment with BPS.

Required Forms

Completed paperwork must be submitted in-person to the Human Resources Department, along with a valid photo ID and proof of authorization to work in the US (See I-9 form instructions for allowable forms of identification). The Human Resources Department is located in the School Administration at 644 Pleasant Street, Belmont.

A background check must be completed prior to starting work in the District. If you have completed a fingerprint-based background check as a condition of employment in a Massachusetts school district in the past year, you may request that district to send a letter of suitability for employment to BPS Human Resources.

You may download and print the required personnel forms from the links below. Forms are also available in the Human Resources office.

You will also receive an email from our payroll provider, Harpers, inviting you to create an account on their system and electronically fill out these required payroll and tax forms:

  • W-4 Federal and M-4 State Income Tax Withholding forms

  • SSA-1945 Social Security Statement acknowledgement

  • Direct Deposit Authorization Form

    • You must also upload an official printout from your bank showing your name, routing number, and checking account number (e.g., voided check, or bank's direct deposit form available from your bank teller or online portal)

  • W-9 Taxpayer ID Number form

    • A W-9 Form must be submitted in order to receive tuition reimbursement

    • Non-us citizens should submit Form W-8BEN instead of Form W-9

  • Retirement Forms:

    • Staff participating with the MTRS or OBRA retirement plans will complete the enrollment form online

    • Town of Belmont Retirement participating staff will need to visit that office in person to enroll

The Harpers Payroll email will come from no-reply@employeeforward.com. Please complete the online payroll forms as soon as possible – you will not get paid unless and until you complete this process, which is described in more detail here: Harpers Payroll Services Onboarding Process

Required Training

New hires must complete all district required training within 30 days of their hire date.
