BPS COVID-19 Chenery Vaccine Clinic Reminder

COVID-19 Vaccine for students 12 and older REMINDER

Dear Belmont Families,

We are happy to remind you that we are working with VaxinateRx and Healthcare Family Pharmacy to offer the 2nd COVID-19 vaccination clinic.  VacinateRx will offer 2 doses of Pfizer COVID-19 vaccination series to our students' ages 12 and up. 

WHEN: Thursday, October 21, 2021

WHERE: 10:00 AM - 1:00 PM, in the Chenery large community room.

All students will be vaccinated free of charge regardless of insurance coverage. If insured please enter insurance information in the online registration. Once registered online there is no need to bring an insurance card to the clinic. 

IMPORTANT THINGS TO KNOW

  • Please register your child to receive the vaccine at Appointment Quest Scheduler
  • After registering you will receive an email with a link to the consent form for vaccination (please check your spam folder if you do not receive it)
  • Please fill out and print the consent form to bring to the appointment
  • For anyone under the age of 18, a consent form signed by a parent/guardian must be presented in order to receive a vaccine
  • Parent/guardian does not need to be present as long as the child has a signed consent form

Your student must have the following documents with them in order to receive the vaccine:

  1. A completed and SIGNED Consent Form. A parent/guardian signature is required for any student under the age of 18.

  2. If this is your student’s second dose, they must bring their COVID-19 Vaccination Record Card. 

The vaccine is free for everyone regardless of insurance coverage

VaxinateRx and Healthcare Family Pharmacy will be administering the Pfizer COVID-19 vaccine which is currently approved for ages 12 and up

Students who received their first dose of the Pfizer series at another location are welcome to register to receive their second dose at this clinic as long as it has been at least 3 weeks since their first dose but no longer than 42 days (per DPH). Please remember to send the vaccination card from the previous vaccination and a signed consent form to the school with your student.

Students are welcome to register to receive their first dose at this clinic

.Students will receive a vaccination card when they receive their vaccine. Please keep it in a safe place.

If you experience any issues with the registration process please email info@VaxinateRx.com  to resolve any issues. 

Reminder: Please make sure to print and sign the consent form and have the student bring it to school on the day of the clinic. A Parent or Guardian signature is required for any students under the age of 18.  

Your child will receive a COVID vaccine card at the time of vaccination after the first dose. PLEASE KEEP THIS CARD IN A SAFE PLACE. Your child will need it for the second dose.
