Second COVID Vaccine Clinic for Students Ages 12 and Older 

Dear Belmont Families,

We are happy to announce that we are partnering again with Pelmeds Pharmacy to offer a second clinic for the Pfizer COVID-19 vaccination to our students ages 12 and up. Their team of vaccinators will come to our schools to administer the vaccine.

When:           Monday, June 14, 2021

Where:         Chenery Middle School 8:00 am - 2:00 pm

                      Belmont High School 8:00 am - 11:00 am

If you are a first-time user of the PelMeds system: After registering, you will receive a link with a username and password. Click on My Vaccine in the email and Login, and you will be brought to a screen where you will input your student’s information. Click Next and then Print and complete the consent form.

If you are a second-time user of the PelMeds system: You will schedule your student’s appointment using the appropriate link below. You will receive a confirmation email for the scheduled appointment. You must print and complete the consent form.

Your student must have two documents with them in order to receive the vaccine:

  1. Signed Consent Form. A parent/guardian signature is required for any student under the age of 18.

  2. If this is your student’s second dose, they must bring their COVID-19 Vaccination Record Card.

PelMeds Pharmacy has indicated that there will be plenty of vaccines for everyone. Additional appointments will be added should all of the time slots fill up. Please do not rush to register immediately after receiving this notice. If everyone attempts to register at the same time it will crash the site (unfortunately this has happened). 

All students will be vaccinated free of charge, regardless of insurance coverage. If insured, please enter insurance information in the online registration. Once registered online there is no need to bring an insurance card to the clinic. 

Students who received their first dose of the Pfizer series at another location may register to receive their second dose at this clinic as long as it has been at least 3 weeks since their first dose, but no longer than 42 days (per DPH).
