2020 Belmont High School Physical Exams for Fall Sports

Dear Families,

The 2020 Belmont High School Fall Sports sign-ups are here! 

Students participating in fall sports must have an updated physical exam on file with the school nurse prior to sign-up and tryouts in order to participate in school sports. 

Per the State Department of Public Health regulations and RULE 56 of the MIAA HANDBOOK, physical exams are valid for 13 months from the date of the exam. Please note that No Exceptions Have Been Made at This Time. Physicians are aware of these requirements. If you are in need of a physical exam, please reach out to your physician. Paperwork must indicate that a student is cleared to participate in activity/sports. A student’s eligibility will terminate once a physical has reached the 13-month limit.  

Students without a valid physical exam on file with the school nurse will be EXCLUDED from sports. Do not allow physicals to expire as we cannot guarantee that the paperwork will be processed in a timely manner during the summer months. PLEASE PLAN AHEAD!  

On June 10th, an email will be sent to all student’s parents and guardians, with instructions on how to access the Belmont Public Schools Athletic Forms. The forms will indicate when your student’s physical exam will expire. Please make note of the date and plan accordingly. All physical exams are to be sent directly to the Belmont High School Nurse by July 31st.

All documentation submitted must be legible. Please keep this in mind when you submit copies of your physicals to the nurses.
